How to ask for and deposit funds
If you need to get reimbursed for an expense, or you need to submit money for a deposit:
Download the appropriate form below
Fill out the form, being sure to select the line item that it applies to (click here for a list of the income/expense line items)
If this is a reimbursement request, route to the PTSA President for approval
If this is a deposit, be sure to have another PTSA member co-sign for you and route to the PTSA Treasurer

W-9 - Please have any vendor who will be paid more than $600.00 in a calendar year complete and return this form
Contractor invoice - Please have vendor fill out this form and return for payment
